A Senior Forms Designer with more than ten years of experience in designing data collection forms for clinical trials. Specialised in converting complicated forms into easy to complete, professional looking case report forms, mainly for phase IV marketing studies.

WORK EXPERIENCE
Senior Forms Designer - Merck Sharp and Dohme Europe, Inc. (1992 – June 2006)
Designed customised case report forms (paper-based data collection tools) for worldwide clinical trials.
Redesigned existing case report forms as part of a streamlining initiative, to follow good design practices and allow reusability. Created a format to handle the different versions of these forms and a new layout that was preferred over the existing style.
As communication specialist supported the implementation of Electronic Data Capture (EDC) for clinical trials. Designed a logo for this initiative, developed a website to share information throughout the implementation process and designed multiple flow charts and visual aids to explain the processes.
Coordinated the design and translation of patient administered forms such as diary cards and questionnaires, dealing with special non-western characters for some languages.
Ensured adequate use of the international format and guidelines for studies done worldwide - being the only non-US based designer.
Coordinated the centralised printing of workbooks, questionnaires and diary cards with external print companies. Selected, contacted and negotiated services/rates with several print companies, compiled a frame contract and implemented new procedures for printing.

Trained and lead a platoon of 40 men and coordinated all outside NBC manoeuvres.
Merchandiser - Procter and Gamble (1989 – 1990)
Responsible for setting up displays to promote the new boys-girls Pampers in all supermarkets and medium-sized local shops.
